Description

I have a six page application form to implement. You can see the first page here:

The site has been implemented on a modx framework and each page of the form has already been created as a html chunk.

I need someone to rig up this form (preferably using AJAX so that the whole page doesn't have to be refreshed when moving to the next/previous page) and have the data saved to a database table. Form validation (Javascript and PHP) is required.

This task needs to be tested and finalised by 11am Monday, Brisbane (Australia).

why not to place the form in iframe or open it as pop up?it will be $100 and 4 days with ajax
