Dear Coders,

This will be a quick and small project for the seasoned AJAX/PHP/JScript JQuery or mootools coders.

Please bid only if you know what you are doing and if you can get it up within 5 days.

Main booking page must have links of icons to 4 options,
Trip-based, Time-based, Long-term, send trip data to us (excel, words, pdf or textfile)

Once clicked on the options, a Dialogs (modal, light weight, lightbox) with the form should appear. required fields for the forms are shown below. once submitted, the data on the form will be mailed to an email address with all the information entered at the same sequence.
