Description

Need dynamic form using Ajax, JavaScript and PHP. I have already started the process, just stuck on adding more variables. Attached are files used and link of what I have created already is here:

WHAT I NEED - Dynamic Form Flow

1. User Select Origin State
2. Based on state selected use ajax to gather information on loads with that origin state slected.
a. Need total loads available
b. Need pull down with available origin cities. But optional,
not a required field.
3. Bases on origin state selected and or origin city selected display pulldown with available destination states.
a. Need pull down with available destination cities. But optional
not a required field.
4. Based on Origin and Destination information generate pull down of available trailer types for that search.
5. Get all information and create search and load listing. This page is already created. Something like this: http://www.rightnowloads.com/full-truckload.php?state=NE

I have created most of the setup. I have a small budget for this project $75 and under.
