Description

I have two sites that I am building directories for and I need a database created to allow users to search by zip code. I need it to work from a database that I can add to with an excel spreadsheet and from someone uploading their profile directly from the site. When they upload their profile it needs to send me an email with the information received from the form they filled out. Nothing too complicated. Need to be easy to use and need to have the ability to use on two different sites. Reference of what I need can be found at cosmeticdermatology.net
