Description:	THIS NEEDS TO BE DONE IMMEDIATELY AND COMPLETED WITHIN 8 hours. PLEASE VIEW ATTACHED DOCUMENT FOR FULL DETAILS. ONLY RESPOND IF YOU CAN DO THIS IN PHP AND IF YOU CAN WORK IMMEDIATELY. YOU will create a new table on our database and the registrations will write to this table. YOU will do all coding and create pages based on our designs (see attached document) Text should run from a CSS file and should be arial 12pt (must run from a CSS file) PAGE 1: An intro page with 4 items that can be ordered (no payment). Customer must enter registered email address to be able to order items. If not registered then an error says "THIS EMAIL ADDRESS IS NOT YET REGISTERED: PLEASE CLICK JOIN TO REGISTER" If registered then thankyou page comes up saying Thank you. Your order is being processed and should be with you within 28 days. PAGE 2: REGISTRATION PAGE This page has a number of fields (some mandatory) Customer must fill out these fields to register. Hyperlink to www.nidp.com When submit is successful, a page and message comes up: "Thank you for registering. If at any point you wish to unsubscribee..." "CLICK HERE to order your packs" CLICK HERE will return to the home page. PAGE 3: UNSUBSCRIBE PAGE This page has a field to enter email address and the user will be unsubscribed from the service. Ie. their details will change on the database table from SUBSCRIBE to UNSUBSCRIBE Upon successful submit, a message comes up confirming unsubscribe. PAGE 4: TERMS & CONDITIONS We will fill this in.
