Description

I have created the artwork for a form and this now needs to function in the following ways:

(a) • All information must write to a TABLE in our database.

(b) • The form must calculate an amount based on 2 values given (type of room and length of stay). There are only 6 alternatives.
1. Single Room Check Out Friday = £48
2. Single Room Check Out Saturday = £96
3. Single Room Check Out Sunday = £203
4. Single Room Check Out Monday = £310
5. Twin or Double Room Check Out Sunday = £118
6. Twin or Double Room Check Out Monday = £236
ANY OTHER OPTION IS £0

(c) The payable amount must appear at the bottom where it says "I understand..."

(d) An email must be sent to the first email address given listing all of the details entered

(e) An email must be sent to the second email address given informing them that the person has requested to share a room with them.

UNDER - YOUR PERSONAL DETAILS:

(f) ALL FIELDS MUST BE COMPLETED EXCEPT:
Second Line of address
Mobile Number must start 07 and must contain exactly 11 digits
Valid From and Valid To must be in following format 09/01/84
Postcode must be no more than 8 characters

UNDER - NAME AS IT APPEARS IN PASSPORT
IF CHECKBOX CHECKED - no name required
IF CHECKBOX NOT CHECKED then FIRST NAME AND LAST NAME must be entered

• Email address and confirm email must match

UNDER - YOUR FLIGHT DETAILS:
If either I AM BASED IN EDINBURGH or I AM BASED IN MANCHESTER are checked then NO FLIGHT MAY BE SELECTED.
If neither of these 2 options are selected then FLIGHTS MUST BE SELECTED AS FOLLOWS:

(h) Only one flight may be selected as OPT1
Only one flight may be selected as OPT 2
Only one flight may be selected as OPT 3
These may all be different or the same.

In field SHARING ROOM WITH - if "NAME NOT ABOVE " is selected then "NAME IF NOT ON LIST" must be completed
Email address & Confirm Email must match

UNDER - YOUR CONFIRMATION

Both boxes must be checked
In first line, amount must appear based on customer selections.

ADMIN SECTION

We require an admin section where we can change the following:

1. What is written in each of the 2 emails
2. Add or remove a name in the drop-down list (both lists should be the same)

TIMING

MUST BE DONE IMMEDIATELY AND COMPLETED BY THIS TIME TOMORROW
