Description

Urgent data collection form required today.

You must be able to complete this today - please don't bid otherwise

Roughly 33 questions with various radio button and freeform answer fields.

All questions are mandatory. Personal details is all mandatory free forms fields with the exception of title which is radio button.

Medcial questions have a "yes" /"no radio button question. If Answer is "yes" "detail" should then become a mandatory freeform field.

Terms and conditions needs to link a new window to t@c page.

Java validation for required field for e-mail address.

Submission date must be written to the database.

Each question must write to it's own field in a table on the database.

1. Create table on our database
2. Design and create PHP page
3. Upload and test on our server

Will pay around $30.
