Description

The feed back form is use to allow user to enter a competition or quiz
I have already a system for the competition schedule, the flash form I use for the moment is a basic flash/php application where user from the flash form enter the contest by the flash form sending me an email, the user receive an auto message of confirmation, none of this data are input in database, I create a schedule management for the section

What I am looking for:
A flash form that record the data into a dedicated table on a mysql database, and do the same function than the existing form sending a registration e-mail to me and a confirmation to the user

The flash form component:
1. Text field: First Name
2. Text field: Last Name
3. Text field: Email address
4. List Menu: Country
5. Text field: Town
6. Hidden field: Competition id number
7. Text area: Optional answer (to contest optional question)
8. And a submit button
All field required a validation, none can be empty except the field “optional Answer”

I got all access and knowledge to implement the table on mysql all I need is the flash form and the php files that work with it
I will need the “fla” source file so I can design different form for different contest
