Description

I want to duplicate the functionality on this page (below the header) http://www.aa.org/lang/en/meeting_finder.cfm?origpage=29 and specifically replicate the 6 functions seen in the yellow box.

I need it formatted to match the CSS of my existing site, and so that my chief programmer can integrate it into my PHP-based site, including the Google map interface, that it resides on my server, and she need only link to it.
