Description

We need someone to build a simple form (PHP) where the output is displayed and saved in a .csv file.

The form should exist of:

* Name: (a-z text only)
* E-mail: (email)
* License: (text including hyphens -)
* Miles: (number)
* Gallons: (number)

Validation of the input variables would be very nice.

After submitting the form a Thank You message on a new page should be displayed with the variables submitted.

Output for Admin should exist of 1 .csv file with the submitted data on new lines as follows:

Name;E-mail;License;Miles;Gallons

I think above is pretty standard and straight forward for a good programmer.
