Description: Live betting odds

I am looking to have the index page of my website adjusted so that it can display live betting odds for the selections displayed. The odds themselves should be hyperlinks, which link to the websites, to allow the person to place their bets.

The links should include my affiliate link, which would be ideally be added in some kind of web form.

Please read the attached text document for further details, and then explain to me how you attend to acheive this.

Preference will be given to those who can provide a working demo.
