Need a input web registration form with totaling and Paypal function (Paypal Button).

The above is layout example and function example. This form is for baseball league. We would like the input in 2 columns to reduce vertical height of page.

Web site is html, Linux (no MS Front Page Extension). Apache ASP, SSI, PHP, CGI support. MySQL and PostgreSQL support.
Need data sent to either email (csv), or to MySQL database on the site. If choosing MySQL option, quote must include config of database.

This form will be inserted into existing webpage. Clicking on Pay button will send data to email/database and also to Paypal using Paypal button link. There is no shipping, no tax, just 1 of 4 prices to select and then adding total (1 player, 2 player etc).
