Survey Code Modification

I have a survey style form set up on this web page www.silositecreator.com/app.html

At the moment what happens is that when a user completes all the questions on the survey form and they click the ‘DONE’ button their First Name and Last Name: Valid Email: Address 1:
Address 2: City: State: Zip: Country. Data are parsed to a special database in my shopping cart system.

That part of the function works fine

What I need a programmer to do is this:

I need a script (code / utility) designed that, at the same time the user clicks the ‘DONE’ button besides the data as I described above being parsed to my shopping cart, I also need all the survey results to be send to an email address. And I need to be able to set / edit the email address to my choosing.

Using the survey I set up on the site www.silositecreator.com/app.html as the example, the email I receive should contain the data as I have set out below

BIDDERS NOTE1: There are critical elements in the existing ‘form code’ that are designed to parse data to my shoppingcart. These element cannot be modified.

BIDDERS NOTE2: The code / utility you develop I need to be functional and usable on other survey web pages I set up in the same way in the future.

BIDDERS NOTE 3: The code must be tested by me prior to payment being released

This should be a simple script for programmers with the knowledge. My budget for this is $ 40
