Hi

This is a very urgent project and I need this finished by tomorrow morning!!!

I need to add an Order Form to each individual product that is ordered on the OS Commerce website.

Basically, when someone buys a certain product, they must fill out specific details for each product.

These include:

Book Title
First Name:
Surname:
Girl/Boy:
Age:
Address:
Town:
Friends: (3 Friend Names)

There is 4 different types of order forms and I will provide all the details for you.

So once again, when a product is added to the Cart, they must first fill out the above details before they can purchase it. This will be the case for each product that is added to the cart.
