Description

Hi there, I need an Ajax contact us form written as a module in Joomla 1.5.6

It should work like a shadowbox effect - see: http://extensions.joomla.org/component/option,com_mtree/task,viewlink/link_id,4577/Itemid,35/

Basically on my homepage, I will have a mod_custom module with a link that says: Contact us. when pressed, an ajax contact us form should appear in a shadowbox. It should have the following fields:

Full Name
Email
Message
Captcha
Submit

and should be emailed to my address.
