We need a complain page just like:

We don't need the "Preferred sites" Choice
We don't need the "Gender" Choice
We don't need the "Age" Choice

We don't really need this dialog because we will be setting these things ourselves. We need a dialog where the customer can choose a dollar amount. The choices should be:

$50
$100
$150
$200
$300
$500
$1000

Depending weather the original choice was Banner Ads or Text Ads the user will get either a text ad dialog or a banner ad dialog. We need our pages to mimic that behavior. We also need for the customers to be able to save their ads into this dialog and have it e-mailed to us.

After page three they should go to paypal checkout page.
