Description

I have a form with several check boxes and to make it so at least one has been checked off and the owner gets which one was checked off in an email. Right now they are not required and all check box items appear on the admin's email. I also need you to review the form and make sure it's all coded correctly and no issues that might cause problems.

Need this fixed immediately and budget is $20. If accepted, and agreeable, I will pay you $30 and then you can send me back $10 so we can review.
