Description

I am looking for windows based patient scheduling application. Project must complete within 30 days once detailed requirements is delivered.
Implement using .NET 2.0, C#, MS SQL SERVER 2005.
I need to own the full license and copyright of the implemented product without any limitations.
Same functionality will be implemented in ASP so I am looking for desktop implementation using classes and business objects so web implementation can use same code. Bidder who successfully completes will get future implementation projects.
It is important that the successful provider is helpful, creative and patient who also speaks and writes fluent English. I must be able to contact you via email, messenger and telephone.
Previous work must be able to be viewed!
Regular updates are required as well as fast responses (within 24 hours or less) to queries and changes. It would be useful if you have a secure area for uploads of changes etc. The site should be able to be viewed on your server so it can be seen 'working' before payment is made.
Application code must be free of bugs and I am looking for high quality application. I will be testing this application on your development environment to identify issues/bugs before final payment. After delivering product, I will need to go through extensive testing to find any bugs and bugs needs to be fixed until 3 months after the product is delivered without paying any extra fees. I am looking for 30 days of delivery once agreed upon, so please do not bid if you will not be able to deliver the product or if you are in the middle of other projects.
Requirements must be discussed over the phone. Partial Database model will be provided and needs to be discussed if any changes required. Also discuss flow/functionalities in detail over the phone before starting the project. I will need help in setting up environment on my system for the finished product. I also need online presentation of final product before delivering so I can make sure that final product is based on the requirements.
Following in summary of what I am looking for. Extensive requirement document will be provided later and finished product must satisfy all requirements before payment is made.

1. Role based security to the application.
2. Approximately 50 different forms to display different data.
3. Scheduling window with calendar, analog and digital clocks, block time feature, adding new patients functionalities, find appointment time slots, scheduling feature including new appointments, cancel, re-schedule, delete etc...
4. Reports
5. Legends to display customized colors on appointment window.
6. Logoff after certain time, overbook logic, waiting list and patient reminders.
7. User defined settings for appointment windows.
8. Look and feel must look like agreed upon, looking for MS Access 2007 sample template look.

Please do not bid if you cannot deliver high quality bug free application. Also, if you cannot make the deadline or if you are in the middle of tight deadline on other work, please do not bid.
