I need a collection of affiliate systems' tracking & codes for showing ads with examples of where they can be found.

For instance, for Google Adsense that could be

Some affiliate systems have multiple variants of tracking codes, e.g. Comission Junction. This should be noted as well, and all alternative tracking codes should be present.

I need tracking codes (clickthru URLs with surrounding HTML) and codes for showing ads for the following systems:

The result should be an excel sheet with these fields: affiliate system, tracking code, two sample pages where the code(s) are present. As I said, there can be multiple lines for each affiliate system.
