Description

Looking for a developer who can create a popup confirmation box, that pops up when particular form "<select>" values are selected. I need to be able to define these values fairly easy.

I would like to have it auto popup a moDal window before form submission like - http://jqueryui.com/demos/dialog/#modal-confirmation

Prefer this done in jQuery since its the main framework we're using.

Developer needs to be available on Yahoo, MSN or Gtalk. Need complete withing a few hours
