I need this form modified as follows:

1. If customer selects any country OTHER than US, "zip code" will not be a required field.

2. "Language" is a required field.

3. "Referred by" is a required field.

4. I want all data fields sent by email to one email address.

5. I want all data fields EXCEPT 8 sent by email to A DIFFERENT email address. [the 8 fields are Company, Credit card #, Credit card expiration date, verification code, mailing address, billing address, voice phone, cellular phone]
