Description

I have a web page online with a client health questionaire. Various points are assigned for different responses, and the total points and applicable category are automatically displayed. Many clients wish to print this and I would like to receive a copy by email in pdf format. Project involves a solution to have a submission button on an html page that will connvert to html and mail a copy to myself and client. This would all be done on server - not on clients computer. Solution includes applicable server program necessary and integration of solution into the above web page as an example.
