Description: Joomla 1.5 YooTheme Template Change (Landing Page)

I am looking to have a landing page that when validated against a database will redirect to the my YooTheme Joomla homepage. An example of the first part of my site can be found at, www.kbcins.com.

When you click the register link there are only 3 text fields. I would like to make this the main capture point, so 15 fields are needed. To include a variety of drop-down menus and radio buttons. I then need this form to sync with the already created SQL database attached to the joomla CMS.

This site is insurance related, and only for agents. The landing page is to be used as the main entry point to the site.

The project deadline is completion by Friday. The budget is $100.

Please bid accordingly, and the time frame is V-E-R-Y important. The deadline must be hit. I think it's fairly simple for someone with PHP/ Joomla experience.
