Description:	Hi I am looking to design a site which is for taking information for leads for mortgages etc. This is what I am looking for: A site very similar to: http://ukbestrates.co.uk/ Similar design but not the same. These are the tabs I will require: Mortgage, insurance, pension, investments and mortgage protection. Firstly, in question 2 of ukbestrates (About you section) I want one other question added which says "Preferred contact time?" then a drop down with the options Morning, Afternoon, Evening. Also change "address" to "House number/Street". Add another question called "Area and surrounding" in this there will be a drop down with these options: Chepstow, Newport, Monmouth, Abergavenny, Cardiff, Bridgend, Swansea. In the mortgage tab I want exactly the same information as asked for on ukbestrates.co.uk. In the insurance tab I want the same as mortgage apart from the questions: "How much would you like to borrow? *" "Have You Found a Home? " "What is the Type of Loan? *" But will list Contents and Building insurance. For pensions tab same as previous except on question 1 like on ukbestrates change it to: Are you a smoker? Then option to click yes or no. Then another question: How much do you wish to contribute to your pension every month? (Approx). Then keep question 2 (About you section) the same and get rid of question 3 altogether. In the investments tab I want the same as this: http://ukbestrates.co.uk/loans.php In mortgage protection tab I want in question 1: Age: box to write in numbers Cover required? (Drop down) answers: Unemployment and incapacity, Unemployment only, incapacity only. Current monthly mortgage payments: £ sign with box next to it to write in numbers. Question 2 + 3 can be kept the same. Once they click "Get my quote" they will see a page saying "Thank you for submitting your quote, we will contact you shortly." The mortgage tab will be the homepage for the site. There must be an admin panel too. In this I would like to have the tabs and on each tab I want to be able to click on a name of a person who has filled in the form and be able to see all the questions with the answers they've filled in. It is important to have the exact time and date of quote. Each quote should have a unique ID which will be used in the following table (named: Customer Quotes Table): Quote ID Quote Area Quote Date&Time Allocated To Action Example 1: RM77081030 Cardiff 7/7/08 10:30am None None Please note that quote id is made up of person initials plus date and time. Also note that "Allocated To" comes from the following table (named: Financial Advisors Table): Quote Area Advisor name Advisor SMS Advisor email Allowed Example 2: Cardiff John Taylor 02920777556 Yes Allowed can be either Yes or No to control quote sending also to enable him/her to be allocated in the Customer Quotes Table. So back to Example 1: RM77081030 Cardiff 7/7/08 10:30am John Taylor None Please note that the software should allow up to 20 names per area (which can be any of the following: Chepstow, Newport, Monmouth, Abergavenny, Cardiff, Bridgend, Swansea)) can be setup and can be used in the first table. The Action tab can have one of the following values: None, Send SMS, Send Email, Send Both. Activity tracking history should be kept so we can check the result of sending SMS and emails. Quote Short info: to be SMSed and will contain the following information: Titled: SMS Info: Name, Date of birth, type of service (mortgage, pension, investment, etc. all the other tabs), contact time, postcode. These must be abbreviated to N (name), DOB for date of birth, TOS for type of service, CT for contact time, and P for postcode. An example would be like this: N=John Smith DOB=5.10.87 TOS=Insurance CT=Afternoon P=CF34PU Quote Detailed Info: to be emailed and will contain full information captured from customer form. Titled: Detailed Info: Full details. It sounds a lot more complex than it is, please tell me the price for this and any questions you may have. I need work started on this immediately and I need it completed in as quickly as time as possible. Note: Quote ID & advisor name should be automated and quote should go around the assigned advisor in a equal manor. Thanks
