| VBA developer since 1997. Here is a suggestion: keep Excel spreadsheet and make it user friendly via VBA. It can all be done with VBA: hiding questions from users, making all sorts of reports, emailing, putting a logo, etc., etc. In short my bid is for converting your spreadsheet to an user friendly one. This is hard to quote without seeing the spreadsheet and without knowing the full means of distribution. My initial reaction is to clean up the GUI and keep this in Excel so that no installation package is required. This would also facilitate distribution by email (you indicated you would like an email response, which infers that the client can also receive email in.) Clearly more discussion is needed.

| I believe this application should be a web front end with a database back end to store results. That way, you can pull the data back out in any format, access, excel , word, reporting services, etc. With a web front end, there is nothing to deploy so it will solve both of your issues.

- vendor Gui based on their location/business type response, will build the weighted questions dynamically in a user friendly format with pop up feature.

| This sounds pretty easy, only reason I put it at 14 days is due to the tediousness related to the project. I am assuming you would also like a program to create your own questions too, so you would not have to have someone rewrite the form program every time you have a new question. If you accept my bid, I would work with you to create a clean, modular program with an extremely user-friendly interface for the form "answerers", and create a clean program for you to create/modify questions.
