Description

Create a one page 10 question quiz with the following attributes:

A heading and instruction text
The questions will be numbered 1 – 10 with a short text.
Each question will have five radio buttons arrayed horizontally underneath with a short text description on each end of the row. (Likert scale)

The buttons will have hidden values from 1 to 5 from left to right.
None are checked on start.
Only one answer is allowed per question.

A submit button will cause the following actions:

The equivalent numeric value of each checked button 1, 2, 3, 4, or 5 will be mathematically summed. For example checking all ten of the left buttons equals a score total of 10 but checking all of the left ten buttons totals 50.
The sum results will post to a new frame (e.g. 400 by 500) or posted to a new web page with the score and a detailed explanation of what the score means. I want both posting options.
The frame (new page) will include the option for the user to email the results to themselves.
The frame (new page) will include the option for the user to print the results.

The desired product will be a script-set into which we can place the actual questions and other text using search and replace.

The code will be placed into FrontPage and hosted on a Unix server with CGI, PHP, PhpMyAdmin, Perl, Python, SSI, Curl, Cron, GD, Image Magick, Module Installations, FrontPage Extensions.

The code engine noted above will do everything except the math, posting, and button commands. If you are selected I can actually use the engine and send you that section already completed.

The code must be browser compliant in all 3.0+ versions of Internet Explorer and Netscape.

A working demo of the script is required before payment.
