Need a PDF created according to the specifications here:
http://www.revenue.state.pa.us/revenue/lib/revenue/2007_pa1000_scanband_final.pdf

My company's software will then populate this form for individuals to make paper copies.

The PDF file needs to match the specifications exactly so that it can be accurately scanned by a machine.
