Description:	Looking for layout and graphic design experieence with Adobe Illustrator 10 and QuarkXpress 6.5 to provide map illustrations and text layout of medical practice information for overprinting pre-printed forms. Project Overview: We require quick turn-around on multiple map illustrations typesetting and layout work using Adobe Illustrator and Quark Express as follows: You will create text and illustration artwork for overprinting within two defined variable imprint spaces on the front of preprinted paper forms. In some cases, you will create additional artwork to place the illustrations on the back of the forms. You will be working with supplied text & logos to typeset and layout information about one or more doctors, their credentials, office addresses, phone and fax numbers. If supplied, you will also need to include email and web site information, a provided logo and a medium to long sentence explaining the health insurance plans the doctor(s) accept and possibly caption text for the illustrations. Using the supplied office address information and your own internet resources, when indicated, you will create a unique map illustration that shows the doctor’s office location and how to get there from nearby major highways by showing major streets that connect the office location with the major highways. Additionally, to aide patients in recognizing their destination, if shown on internet resources or indicated in supplied text, you will indicate and label major beneficial landmarks such as rivers, lakes, rail lines, airports, schools, hospitals, parks, bus stops, transit stations etc. These map illustrations do not need to be to scale and in most cases should not be. The level of detail (for example; showing cross streets) should increase in close proximity to the office location. Project Scope: Total number of different artwork layouts required: 5 Total number of map illustrations required: 15 Background: You will be creating the variable overprint one-color artwork to personalize preprinted medical referral forms given to patients by their doctors telling them to obtain specialized medical care from another doctor. You will create varying layouts to fill and fit the available space depending on the number of doctors and office locations for the medical practice who is receiving the referral. The space and layout will be similar to what would go on a crowded stationary letterhead and business card. In most cases you will create one or more map illustrations that will guide the patient to the referred doctor’s office(s). If there is only one location the map illustration will generally be positioned next to the doctor and location information on the front of the form. If there are two or more office locations the maps will usually be printed on the back side of the forms allowing for more doctor and office location information on the front of the form and multiple larger map illustrations on the back side of the forms. We are looking to establish a relationship with someone who can do this artwork and similar artwork for our other forms on a regular basis one or more times per week as needed with quick turn-around on usually smaller size batches. Typesetting Specs: Fonts: Goudy, Helvetica Neue and Cisalpin LT Std for maps File specs/Software: Maps returned as Illustrator 10 eps file Layout returned as QuarkXpress 6.5 Illustration Specs: Text on maps should be as large a possible. When map is scaled down to actual print size avoid resulting type that is less than 8 points whenever possible. Shaded tones limited to 20% max.
