Description
I need the form as pictured in the attachment reproduced in Flash such that I can easily integrate it into an existing project.Attached is a Zip file of a Flash form I really liked because it had error checking built into the Flash. Unfortunately, it doesn't have all of the fields I need.
Attached is an image of what form fields I need and how the form should look. I need it as soon as possible, today if you can.
