Description:	I have a simple flash form which consists of 2 form fields. I need the flash form to be configured so that one can fill in the form fields and can upload multiple pdf/doc files and upon clicking, the form data along with attached files need to be submitted to a particular email id defined in the code. No backend required. I believe that it is hardly half an hour job. Bid it now and take the project. Get it done and get 10+ review. Additional information will be submitted in next 30 mins which will consist of screen shot. You just need to put those two fields in a simple flash and configure it with attachment facility.
