Description

I am a startup company that would like a website designed that my users can access to request to be a client or subscriber. I need the ability for them to complete a demographic form, include a picture and my ability to search on the back end. I will additionally need the ability to link them securely to be able to pay for services through the website. I am looking for about 3~4 pages that are simple however a little flashy. This is a matchmaking company and therefore one of the pages needs to describe the companies services, and then link them to the forms pages.

I forgot to mention I would need assistance with logo and banner design. This website would be strictly to collect data from my perspective clients and to have a space for them to go to too read about my services. It's not a dating website, it is where my clients can provide me their personal contact information in a secure environment. Domestic companies only please and thanks!
