Description

I need a single web page developed with the following requirements:

Flash Banner Design
Embedded borderless flash vide (already completed --- you just need to integrate it into the site)
Web form (14 fields maximum --- cannot invoke client's email system)
Content

I need a professional look and feel. The image files cannot be pixelated.

This is an urgent project so DO NOT bid if you cannot deliver ON TIME.

Also, I am happy to deposit money into an escrow account but I will not pre-pay any services.

You will be paid 50% when the first iteration is published to the production site and is functional.

You will be paid the other 50% after the project is accepted.
