so the key features of the site should be :-

3 - Scheduling for ( Email , Chat, Phone ) Consultations.
4 - Payment Processing ( for doctors and for customers )
5 - blog
6 - Medical Wiki
7 - Chat ( Between Doctors and Patient / Rooms and 1 to 1 - Ability to create more roooms) built into site.
8 - Patient and doctor chat history has to be stored in doctors and patients account.
9 - ability to signon to site ( both Doctor and Patient) and initiate chat sessions.
