Description

I have a 28-page PDF of about 1000 records. The scan is clean, the PDF 10meg. See a sample attached. Records are in the format:

Firstname Lastname
Title
Company
Telephone
Email address

I want the records typed into a spreadsheet. Data should be in these columns:
FirstName|LastName|Title|Company|Telephone|EmailAddress
(Last Name would be the last word in the name field on the record.)

How you do it is up to you; hand-entered or OCR'd. Data must be 99.8% correct however.
