Description

I need to create a simple paper form exactly as the one in the attached picture. It should be in pdf format and be submitted ASAP, possibly within the hour. It was previously created using Adobe PageMaker but I lost the relevant files.
Please do not bid if you are not confident of completing it PERFECTLY and on time. Also, it has to be created from scratch, not by editing the image and printing it to a pdf.
