I would be greatly interested in tackling this project in that I am a former health care worker,( Medical surgical nurse), which I believe in the UK is called a theater sister?? ) who has also written a great many articles and content for Health Community Online. The topic that you offer is quite interesting to me in fact. I would suggest a layout of about 40-50 pages Outlined possibly in the charting format that is used in most hospitals, the problem, the diagnosis,(i.e. learning deficit with regard to proper nutrition etc.) in the following way: Subjective information (reported by the client), Objective information (observed by the practitioner), Assessment (drawn from the data), Plans (for action related to the problem), Implementation (of plan), Evaluation (of plan), Reassessment (of client's needs for further instruction) one to two page introduction to the physician: Moving into Problems: 1. 2. Underlying belief systems and ideas that lead to those problems. Therapies 1. 2. 3. etc Solutions or revisions necessary to the therapy or treatment for individuals.
