1. Thrombophilia: Factor V Leiden mutations
Blood clotting, or coagulation, is an essential part of wound healing. Coagulation occurs by a complex series of interactions between proteins contained in blood. Inherited mutations to any of the blood borne proteins in the clotting “cascade” can mean that blood clotting is abnormally slow (haemophilia) or active (thrombophilia). Abnormally high coagulation (hyper-coagulability) increases the likelihood that an individual may suffer a blood clot.
Factor V Leiden mutations are the most common of the hyper-coagulability disorders, and are in 5% of the Caucasian population and 1.2% of the African American populations. Individuals who have one mutated gene are said to be heterozygous for the condition, whereas individuals with two mutated copies of the gene are homozygous. The risk of thrombosis is elevated by three to eight percent for heterozygotes and by 30 to 140% for homozygotes.
Many individuals, especially heterozygotes may never be aware of the condition and may never suffer a blood clot. However, in combination with other risk factors, such as use of the oral contraceptive pill, surgery, trauma, smoking, dehydration and pregnancy, the risk of thrombosis may dramatically increase. Factor V Leiden mutations are associated with two conditions: venous thrombosis and pulmonary embolism.
Venous thromboses are blood clots that occur in the veins (blood vessels that carry blood back to the heart). Typically, they occur in the deep veins of the lower legs (deep vein thrombosis) or arms, in the veins surrounding the brain (sinous vein thrombosis) or in the intestines (mesenteric vein thrombosis). Small blood clots lodged in deep veins are quite common and may not cause clinical symptoms; however as clots increase in size they begin to obstruct blood flow, causing heat, redness and swelling of the affected area.
As clots grow, they become unstable and may dislodge from the wall of the vein, in a process known as embolisation. These clot fragments are then carried through the blood stream and may lodge in smaller vessels. Typically, clot fragments lodge in the blood vessels of the lungs, where they may cause a pulmonary embolism. This is a potentially life threatening condition and must be treated very seriously.
Other complications associated with Factor V Leiden mutations include trans-ischaemic attacks (stroke), possible stillbirths or frequent miscarriages and pre-eclampsia.
It is possible to test for a Factor V Leiden mutation by taking a blood sample and performing a simple laboratory test. Your general practitioner can perform this test and answer your questions about thrombophilia.
2. Ectopic pregnancy
When an egg is fertilised after ovulation, it travels along the fallopian tube and into the uterus where it implants in the blood-rich uterine lining. In an ectopic pregnancy, the embryo implants outside of the uterus. Often, implantation occurs in the fallopian tube, however in rarer cases the embryo implants in the cervix, ovary, or within the abdominal cavity.
In almost every case of ectopic pregnancy, the embryo will die because blood supply will not be able to nourish the growing embryo. In some cases, the growth of the embryo within the fallopian tube will cause the tube to rupture. This is potentially life threatening situation for the mother.
Ectopic pregnancies occur when the passage of the fertilised egg along the fallopian tube is impeded. If a woman has previously had a tubal ligation or tubal ligation reversal, scarring and therefore ectopic pregnancies are more likely. Similarly, previous bouts of pelvic inflammatory disease or
Ectopic pregnancies may be initially quite difficult to detect. Implantation of the embryo outside of the uterus often means that pregnancy hormones, including human chorionic growth hormone (hCG) and progesterone are absorbed by the body and are not detected by standard urine pregnancy tests. The signs of normal pregnancy, included a missed period, breast tenderness or morning sickness may be present for ectopic pregnancies. A woman may experience abdominal or lower back pain, or abdominal cramps. Vaginal bleeding or spotting may also be present. Bleeding into the abdomen which places pressure on the diaphragmatic nerve may cause a woman to feel pain in her shoulder. A sudden, severe onset of abdominal pain or dizziness, rapid pulse and collapse are associated with rupture of the fallopian tubes.
