Please read carefully before you make your bid.

attached pdf file is one of the documents we need to design. we have almost 50 documents similar to this form. all goverment court documents . You can find all these 50 documents www.ontariocourtforms.on.ca/english/family

we have a program similar to it right now in the market buts its stand alone (.exe) but i would like go with web based.

First of you all i dont really care what database you use, but as long as its capable of doing what iam looking for, i am happy.

first of all i need to have a super user(administrator) account where he can create new accounts and disable some one from log in.

then you have users (lawyers) logs in ( all have their on username and password but in user page, he should have a place to change his password once he sign in for the first time),then he/she add new clients or work with existing clients. when he click on new client button, he should have a place he can put all of his clients information (name, date of birth, defendant lawyer information etc) then he select all the forms(50 of them)he needs for his clents and added to his clients account. Once he do that, he should able to open each forms and add (type in detail information and then he saves the data. he should have an option to edit the data if he choose to.Dont forget that when user open up a form, the core information should be filled in automatically (name, date of birth, defendant lawyer information etc).

then he should be able to print it and should look exactly like the the forms we downloaded from the site. (www.ontariocourtforms.on.ca/english/family)

The key reason we need this program is to reduce the time you type the basic information and edit and save the documents.

Please note that first page English, second page french and so on. So when you fill in the first page, it should reflect the second page with the same information. also, some forms have calculation, so when the put number in, it should calculate. attached document involved calculation and see if you can make this work

Finally , when you print this document, it should print out the same way as it is now. because these are government documents and we are not allowed to make any kind of change to the document design or looks.

I suggest you print this pdf file rather than you look at on the monitor. it will give you more idea if you have a hard copy in your hand. Please note that attached documents is one of the hardest one in the group.. I would like to get an answer in couple of days.
