Description
Need a simple form with small survey, information will be sent to email, no database need. Form will be add in a PHP file.Form will have the following;
First Name
example of survey to add in form (less than 10 questions with several choice to select only 1) and a drop down menu and text for comments.
How long will you stay at our place?:
(select button) 1 week, 2 weeks, 3 weeks
They will have to select each of survey and fill all form to be able to submit, if not will ask them to it.
Also add captcha for input form to avoid spam.
Form will be sent as text to the email adress I have.
After form submit, will be redirected to a page I will choose.
