Description

I need a template HTML email for newsletter and one MySQL database to store relative info.
I need to email newsletters regular for buiness events of my company. So I need someone to design a template email for me to see if my idea work or not.
The main functions are below:
1. The template newsletter's format is HTML.
2. after my customers read the newsletter, in the end of the newsletter, it will show a quiz to let customer to choose and then click "Apply" to finish. (see attached pdf file)
3. after customer click "Apply", the answer will be sent to my web server and save the answer in MySQL database for statistic.
4. so also need a basic MySQL database to store the data got from customer's reply.
5. a basic web page to show the info that got from customers' reply.
it need to show customer's email address, the email-ID, the answer.
The purpose for the templete is to prove the functions work or not so that you do not need to real design a good format for the template. I will give you the web site (1nad1.com hosts my web site) for you to access and create MySQL database and do tests.
