Description

Sign up form for curling club in pdf.
Need 2 javascript written to help calulate entry fees.
1. Five checkboxes (Mon, Tues, Wed, Thurs, Fri) each with export values of 210. If more than one box is selected, then the total is displayed as 390.

2. Script to calculate total column, 7 entries with a maximum total.

Will send the pdf to successful bidder.
