Description

Create Logo Download form for ASV website that requires users to agree to terms of use/service before downloading can begin.

The logo files cannot be downloaded unless the user Reads and Agrees to the guidelines first. This would be similar to installing most new software programs where you have to check the "I agree to the terms and conditions" statement before it will allow you to proceed. I also want the system to write the username, date and time of consent to a sql database table (which we can pull from their login information) and email that information to a staff member.
