Description

I have a pdf form that needs to be adapted so it can be posted to our website so that customers can complete the form and use a submit button to submit it to our sales email address. The form must be received by email in the same pdf format.
We will need to be able to edit the form in the future.
At the minute the non-functioning pdf form can be downloaded from http://www.scubascuba.com/maldives-downloads.asp and it's the booking form...
