Description

I already have the form created, I just need someone to get it working. Site is running in windows and hosted at godaddy.com. Its an ASP form. I'll give you temp access to download the existing page with the form so you may edit it. I have a second form on my site I'll need working as well wich is the same as the first but with less options.
