Description

A current site is set up in .ASP and is on their server.

They want a claims section/form new page that is a link from a graphic from an existing page.

We would then like a questionnaire to be brought up (similar to the accident advice helpline’s http://www.accidentadvicehelpline.com/) so that the client will input their name and contact number

We would like the following tick box options to be given to the
client:-

• Head
• Neck
• Back
• Arms
• Hands
• Legs
• Feet
• Psychological injuries

When the client has clicked submit we would like the appropriate claim form to be displayed (File is called ‘Claim Form’) depending on which one they tick i.e. if they tick ‘foot’ and ‘hand’ then the valuation for foot and hand injuries will be displayed.
