Description

Hello, this is for a website perfectiondesign.net, there are 3 forms one you will see on the contact page, all I want is this form to post in my email address on a very plain and simple way, secondly you will see another form under the graphic on your right called push to talk, again this is a simply form that I want to be posted on my email address on a very plain and simple way. The third form is a bit different, I want this form as well to be posted in my email address but once a person fill it and click send, not only it comes in my email but as well it sends an email confirmation to who ever filled it with a confirmation number that is generated automatically with a nice message saying that we have received their email and that we will get back to them shortly. What ever number its not important between 4 to 8 numbers. All I care is that those numbers always increase, so if the first time its used the first ever number is 0001 then the second time it will be 0002 and so on. This is just a refference number that is it, no database or nothing is kept in file what so ever.

Now on that form you will see on the support page I would like to add 2 things, 1 radio button set that says are you a current client yes or no. then 1 check box with the word beside it saying emergency call. This box can be checked or not depending if who ever fills it wants be to know if its important or not.

Again I did put a lot of information above but its very simple, 2 forms that simply go in an email, no database or file are kept. Third form does the same but it simple generate a simple number for refference only and send an email back to who filled the form.

hi i can make simple e mail templates which are sent from different forms with the form data filled form data will be replaced from templates when a form is submitted and sent so it is easier to edit the email content without touching the code let me know thanks anand
