Description

We require the following forms to be made in javascript / php or asp.

Our servers are windows based but they also support php. Form details should be mailed to us on 2/3 different email addresses. Form should look exactly similar to what has been shown in the above links i.e in color, font size, font, buttons and positioning.

We require the IP address of the sender to be sent along with the date and time

Form should be made in such a way that no spammer can fill in details to harass. We dont want to pay anything more than 30$.
