Description: I have a MS FrontPage form that is not working.

If the problem can not be resolved, I'd like to convert
the form to ASP. The confirmation page shows
confirmation fields, http://www.nextgensoftware.com/about/infoconf.html.
This worked in test, so I don't know why it is broken now.

Pretty simple job. You should give more details such as where you want the data going. I would assume a database. I can set that up, and also setup an automatic e-mail response to the email they entered, thanking them. And also a password protected page where you can view and/or manage your data.
