Need Help with PHP forms in flash website

Hi, I have a flash Website where I need to fix two forms. Both of these forms are done in PHP. These forms need to be fixed in order to get all information submitted to my specified email address. All information that goes to my email I want it to be organized, not just a bunch of text all together in a paragraph, has to be separated and easy to read. I have a contact form ( only 4 fields ) and a Quote forms ( for Web design estimates. ) I only these two forms fixed, if you have a better script that I can use, please go ahead and suggest, am open for ideas. I need this done right now so please only bid if you have time to do this today. I would like to pay for project when project is finished and when you can show me that everything works perfectly, not before. People with no feedback please do not expect me to put escrow.
