Description:	I need a module for Drupal which is a Form that captures all user input data and stores in the database. Data could then be viewed and changed via the admin of Drupal. Would be nice if this was a plugin that could be installed and configured. Meaning....i would be able to create the form / edit the form via the admin. Example of the form would be Name Address Phone number Comments Items like that. The form will have checkboxes, select boxes, radio and text field and entry boxes. There are about 20 fields. Again....if there is already a module that can be installed to do this.....just pointing me in that direction and how to install it will count as completing the project....this does not have to be written from scratch if something already exists. Budget for this is around 50.00
